If in game description is said for example: Finished by 42 users who rated the difficulty as medium. Is it means that it has been finished by 42 users from which most of them rated it as medium, or it had finished by more than 42 users from which 42 stated that the game is medium?

the 42 users are "registered users". All of 'em gave a rate (to play again), and the mean value from the 42 rates makes the game rate... (well, probably not the mean value, but the rate that has been given the max votes)
